Funder

The Global Fund to Fight AIDS, Tuberculosis and Malaria (through the Center for Collaboration in Health)

Overview
2018–2020

RARE-HIV-TB is a three-year, community-based project implemented by Pathfinder in the provinces of Gaza, Manica, and Sofala. It works to contribute to the reduction of tuberculosis incidence and reducing the TB mortality rate, as well as reducing new HIV infections and HIV-related deaths.

Project Activities

  • Conducted home visits to increase ART retention and support newly-enrolled patients on ART.
  • Identified risks of loss and support to defaulters and follow-up of patients.
  • Disseminated information regarding existing support groups (Community Adherence Support Groups, Mothers to Mothers, Men to Men, etc.).
  • Empowered community-based organizations to manage these groups within the community and improve linkages with health facilities.
  • Promoted male uptake of health care services through home visits.
  • Promoted patient empowerment through the improved dissemination of information regarding human rights, patient rights, and responsibilities and the humanization of health services.
  • Conducted community dialogues to engage and educated the community, traditional leaders, and religious leaders about HIV literacy, TB, treatment, retention, adherence, access to health care, and issues of stigma, discrimination, and their solutions.
